But you forgot to include 'halfway-clued management' - without that you haven't got a hope in hell of even getting three 9's. ;)
Covered under 2 (above). You need clued mgmnt even to ask for it, let alone "pay" for it. Non-clued management gets ingress filtered at the initial cost estimate layer, for cost-estimate layers that are properly operational.
